Provider IBD Digital Notification Guide for Trellus Elevate™

This guide was designed to ensure you have all the necessary tools you need to partner with us in delivering the highest quality of whole person support to you and your patients with IBD.

​Understanding the complexities of managing IBD, our co-founders developed an automated system to help providers stay updated on best practice recommendations and quality metrics. As a Connected Provider, you’ll receive notifications based on patient tracking, giving you a view of your patients’ progress between office visits. The system is designed to educate patients on effective communication with their IBD provider team, easing the workload on you and your staff, with additional support from the Trellus Elevate™ Team.

​The Trellus Elevate™ digital notification and alert system was developed to ensure that your patients receive timely notifications when reporting poor or worsening health trends. This enables them to communicate with your team in real time, potentially avoiding unnecessary ED visits. Additionally, patients will be reminded of required lab monitoring based on their IBD medications and health maintenance due dates. This educational self-management approach empowers patients to safely adhere to their therapies and maintain tight control over their disease.

Red Flag notifications are triggered when your patient is reporting some poor health trends, including clinical and well-being outcomes, and or unplanned health care utilization which requires attention. Your patient will be advised to connect with you and your team to discuss this change in symptoms and next steps.

Yellow Flag notifications are triggered when a patient is reporting some poor health trends, including clinical and well-being outcomes, as well as key biomarkers of inflammation indicating there is rising risk of health care utilization. Your patient will be advised to connect with you and your team to discuss this change in symptoms and next steps.

These include labs, well-being and symptom burden triggers. The Trellus ElevateTM Team will work with your patients to support them, and patients will be directed to their providers if symptoms worsen, or other red or yellow flags are triggered.

Health Maintenance and Disease Monitoring Reminders

TrellusElevate™ simplifies medication safety monitoring and preventative are/health maintenance with automated best practice reminders that notify your patients when there are actions to be taken. Connected Providers will have centralized access to the reminders on the Trellus Provider App.

When adding new therapies or reviewing current medications, you can refer to our Glossary of Medication Monitoring and IBD Vaccine Reference Guide accessible in the App . We also provide these resources to your patients so they can increase their understanding of why these monitoring reminders are important and help to meet national standards.

TrellusElevate™ facilitates precise disease monitoring by leveraging clinical disease severity assessments completed by your patients upon enrolling in the program. If a patient is missing or overdue for biomarker tracking in Trellus​ Elevate™, they may receive notifications prompting them to request a lab order from their provider. The objective is to empower your patients to proactively manage clinical disease activity, enabling informed discussions with you about optimizing treatment based on objective biomarkers.

Vaccine recommendationDue for Flu ShotGetting the flu shot is the best way to protect you from getting the flu. Remember to get the flu shot ( non-live) and not the nasal mist (live vaccine). If possible, members of your household should also opt for the flu shot and avoid the nasal mist. It is recommended to speak to your GI or primary care provider for more information. As a reference for you, we’ve created an IBD Vaccination Reference Guide.
Vaccine recommendationStarting Rinvoq®Getting the Shingrix (Herpes Zoster) vaccine is the best way to protect yourself against getting shingles when you are on certain IBD therapies (Xeljanz®, Rinvoq®, Zeposia®) above age 50 or above the age of 18 and are or will be at increased risk of Herpes Zoster due to immunodeficiency or immunosuppression caused by known disease or therapy. Shingrix is an inactivated (non-live) vaccine. It is recommended to speak to your GI or primary care provider for more information.
